Background: Dyslipidemia is a significant factor in the development of Coronary Artery Diseases (CAD). This study aimed to evaluate these new indices and other risk factors for CAD using angiographic data from an Iranian population.
Methods: This analytical study utilized data from patients who underwent angiography at Razi Hospital in Birjand (Southeast Iran) and who were registered in the National Cardiovascular Disease Registration System. A total of 2,890 individuals were included in the study, comprising 1,888 with CAD and 1,002 without CAD. In addition to evaluating traditional lipid profiles, novel lipid indices including Castelli’s Risk Index I (CRI-I), Castelli’s Risk Index II (CRI-II), Atherogenic Coefficient (AC), Triglyceride-Glucose Index (TyG), and Atherogenic Index of Plasma (AIP) were also analyzed using descriptive and analytical statistics in SPSS version 16.
Results: The results showed that CAD patients were older (mean age of CAD 63.46 ± 11.43 years, mean age of  non-CAD 56.74 ± 11.80 years, P < 0.001). The proportion of males in the CAD group was significantly higher (68.5% vs. 42.1%, P < 0.001). Notably, novel lipid indices such as CRI-I (OR: 1.196; 95% CI: 1.117-1.279), CRI-II (OR: 1.273; 95% CI: 1.168-1.388), Non-HDL (OR: 1.004; 95% CI: 1.003-1.006), AC (OR: 1.197; 95% CI: 1.118-1.282), and TyG index (OR: 1.298; 95% CI: 1.161-1.452) were significantly higher in the CAD group compared to the control group. In contrast, variations in AIP or triglycerides did not show significant differences between the two groups (P =0.68)(P=0.3)
Conclusions: The results of this study show that novel lipid indices, especially CRI-I, CRI-II, AC, non-HDL cholesterol, and TyG index, have a significant relationship with the presence of CAD and can be used as complementary tools, along with traditional factors such as age and gender, to better predict and identify CAD in the Iranian population; while the AIP index and triglyceride level did not show independent predictive value in this regard. Therefore, designing preventive programs tailored to the characteristics of each population seems essential.
