Background and Aims: Dyslipidemia is one of the important factors in the occurrence of coronary artery disease (CAD). Therefore, this study aimed to evaluate new lipid indices and other risk factors associated with CAD.
Materials and Methods: This analytical study used data from patients undergoing angiography at Razi Hospital in Birjand, Iran. The study included 2890 subjects: 1888 coronary patients and 1002 non-coronary patients. Traditional lipid profile and modern lipid indices, including Castelli Risk Index Type I and II (CRI-I) and (CRI-II), atherogenic coefficient (AC), triglyceride-glucose index (TyG), and plasma atherogenic index (AIP), were examined and analyzed in SPSS (version 22).
Results: Coronary patients were older than the non-coronary group (63.46 ± 11.43 versus 56.74 ± 11.8 years; P<0.001). Also, the proportion of men in the coronary group was significantly higher (P>0.001). New lipid indices included CRI-I (odds ratio: 1.196; 95% confidence interval: 1.117–279.1), CRI-II (odds ratio: 1.273; 95% confidence interval: 1.168–1.388), non-high-density lipoprotein (HDL) cholesterol (odds ratio: 1.004; 95% confidence interval: 1.006–1.003), AC (odds ratio: 1.197; 95% confidence interval: 1.118–1.282) and TyG index (odds ratio: 1.298; 95% confidence interval: 1.452–1.161) were significantly higher in the coronary group. No significant difference was observed in AIP index and triglyceride levels between the two groups (P=0.68) (P=0.3).
Conclusion: Novel lipid indices, especially CRI-I, CRI-II, AC, non-HDL cholesterol, and TyG index, can be used as complementary tools to better predict and identify CAD in the Iranian population, along with classical factors, such as age and gender, while AIP index and triglyceride level did not show independent predictive value. Therefore, preventive programs should be designed according to the characteristics of each population.
