چکیده:   (354 مشاهده)
زمینه و هدف: دیس‌لیپیدمی یکی از عوامل مهم در بروز بیماری عروق کرونر (CAD) است. لذا این مطالعه با هدف ارزیابی شاخص‌های نوین لیپیدی و سایر عوامل خطر مرتبط با CAD انجام شد.
روش تحقیق: این مطالعه تحلیلی از داده‌های بیماران تحت آنژیوگرافی در بیمارستان رازی بیرجند استفاده کرد. ۲۸۹۰ نفر شامل ۱۸۸۸ بیمار کرونری و ۱۰۰۲ فرد غیرکرونری وارد مطالعه شدند.
پروفایل لیپیدی سنتی، شاخص‌های نوین لیپیدی شامل شاخص خطر کاستلی نوع
I و II (CRI-I) (CRI-II)، ضریب آتروژنیک (AC)، شاخص تری‌گلیسیریدگلوکز (TyG) و شاخص آتروژنیک پلاسما (AIP) در نرم‌افزار SPSS نسخه 22 مورد بررسی و تحلیل قرار گرفتند.

یافته‌ها: بیماران کرونری سن بالاتری ازگروه غیرکرونری داشتند (11/43±63/46 در مقابل 11/8±56/74 سال؛ 0/001>P). همچنین نسبت مردان در گروه کرونری به‌طور معناداری بیشتر بود (0/001>P). شاخص‌های نوین لیپیدی شاملCRI-I  (نسبت شانس: 1/196؛ فاصله اطمینان 95٪: 1/1171/279)، CRI-II (نسبت شانس: 1/273؛ فاصله اطمینان 95٪: 1/1681/388)، کلسترول غیر-HDL  (نسبت شانس: 1/001؛ فاصله اطمینان 95٪: 1/0031/006)، AC  (نسبت شانس: 1/197؛ فاصله اطمینان 95٪: 1/1181/282) و شاخص TyG  (نسبت شانس: 1/298؛ فاصله اطمینان 95٪: 1/1611/452) در گروه کرونری به‌طور معناداری بالاتر بودند. تفاوت معناداری در شاخص AIP و سطح تری‌گلیسیرید بین دو گروه مشاهده نشد (0/68= P)(0/3=P).
نتیجه‌گیری: شاخص‌های نوین لیپیدی به‌ویژه CRI-I، CRI-II، AC، کلسترول غیر HDL و شاخص TyG می‌توانند در کنار عوامل کلاسیک مانند سن و جنس، به‌عنوان ابزارهای مکمل برای پیش‌بینی و شناسایی بهتر CAD در جمعیت ایرانی مورد استفاده قرار گیرند؛ در‌حالی‌که شاخص AIP و سطح تری‌گلیسیرید ارزش پیش‌بینی‌کننده مستقلی نشان ندادند. از این رو طراحی برنامه‌های پیشگیرانه متناسب با ویژگی‌های هر جمعیت ضروری به نظر می رسد.
