fa بررسی میزان اثر کلونیدین در جلوگیری از بروز لرز به دنبال بیهوشی عمومی بيهوشي Anesthesiology مقاله اصیل پژوهشی Original Article <p align="justify">&nbsp;<strong> زمینه و هدف: </strong>یکی از عوارض ناخوشایند بیهوشی عمومی، لرز است که در 40% موارد دیده می&rlm;شود. لرز عارضه&rlm;ای بسیار جدّی است که مصرف اکسیژن را تا چند برابر بالا می&rlm;برد؛ همچنین باعث افزایش تهویه ریوی و برون&rlm;ده قلب، فشار داخل مغز و چشم و نیز افزایش میزان درد بعد از عمل و خطر ایسکمی میوکارد می&rlm;گردد. پیشگیری از بروز این عارضه ناخواسته برای دستیابی به یک بیهوشی مطلوب و کاستن خطرات آن ضروری است. این مطالعه با هدف تعیین اثر کلونیدین در جلوگیری از وقوع لرز بعد از بیهوشی عمومی انجام شد.</p> <p align="justify">&nbsp;<strong> روش تحقیق: </strong>این مطالعه به صورت کارآزمایی بالینی دوسویه کور و تصادفی بر روی 100 بیمار بالای 18 سال با ASA کلاس 1 و 2 کاندید عمل جراحی چشم در بیمارستان الزهرا زاهدان انجام شد. بیماران به صورت دسته&rlm;بندی ده&rlm;تایی به دو گروه مورد (کلونیدین) و شاهد (دارونما) تقسیم شدند و دو ساعت قبل از بیهوشی 2/0 میلیگرم کلونیدین یا دارونما را با 30 سی&rlm;سی آب دریافت کردند. روش بیهوشی در هر دو گروه یکسان بود. همه افراد در اتاق ریکاوری از نظر وقوع لرز معاینه شدند و پرسشنامه مخصوص تکمیل گردید. داده&rlm;های جمع&rlm;آوری شده با استفاده از نرم&rlm;افزار SPSS (نسخه 13) و آزمون آماری کای&rlm;دو، در سطح معنی‌داری 05/0> P مورد تجزیه و تحلیل قرار گرفتند.</p> <p align="justify">&nbsp;<strong> یافته‌ها: </strong>از 50 بیمار گروه مورد (دریافت&rlm;کننده کلونیدین)، فقط یک نفر (2%) و از 50 بیمار گروه شاهد (دریافت&rlm;کننده دارونما)، 10 نفر (20%) دچار لرز بعد عمل شدند که این تفاوت از نظر آماری معنی&rlm;دار بود (004/0 =P ) .</p> <p align="justify">&nbsp;<strong> نتیجه‌گیری: </strong>این مطالعه نشان داد، کلونیدین در پیشگیری از لرز بعد از بیهوشی مؤثر است و می&rlm;توان از آن به عنوان یک داروی قبل از عمل به طور روزمره استفاده نمود .</p> Background and Aim: Shivering is an unpleasant complication of General Anesthesia which is seen in 40% of cases. Shivering is a serious complication that increases oxygen consumption to several times and results in increased ventilation, cardiac output, myocardial ischemia, intraocular & intracerebral pressure and intensity of postoperative pain. Preventing this unwanted complication is necessary to reach perfect anesthesia and to decrease its hazards. Materials and Methods: This study was performed as a double-blinded randomized clinical trial on 100 patients demanding eye surgery, aged over 18 years old, having ASA class I and II in Alzahra hospital of Zahedan. The patients were randomly divided into two groups using blocking method: Group A received Clonidine and Group B received placebo. Before anaesthesia, each of the cases and controls received 0.2 mg of clonidine and placebo with 30 ml of plain water, respectively. General anesthesia techniques were the same for both groups. In the recovery room, all the subjects were assessed for shivering and special questionnaires were filled out. The obtained data was analyzed through statistical analysis by SPSS version 13 software, using chi-square test, at the significant level of P<0.05. Results: It was found that only 2% of the cases but 20% of the controls developed shivering the difference was statistically significant (P=0.004).
