fa بررسی الگوی تجویز منطقی داروی ایمونوگلوبولین وریدی (IVIG) بر اساس اندیکاسیون‌های پذیرفته شده سازمان غذا و دارو (FDA) در بیمارستان‌های آموزشی بیرجند Evaluation of rational prescription pattern of intravenous immunoglobulin (IVIG) based on FDA-approved indications among teaching hospitals in Birjand فارماكولوژي Pharmacology مقاله اصیل پژوهشی Original Article <div style="text-align: justify;">زمینه و هدف: از بی‌خطرترین محصولات بیولوژیکی می‌توان به ایمونوگلوبولین وریدی (IVIG) اشاره نمود که در درمان بسیاری از بیماری‌ها اثر دارد. مطالعه حاضر به منظور بررسی الگوی تجویز منطقی داروی IVIG بر اساس اندیکاسیونهای پذیرفته شده سازمان غذا و دارو (FDA) در بیمارستانهای آموزشی بیرجند انجام شد. روش تحقیق: در این مطالعه توصیفی-تحلیلی، جامعه مورد مطالعه ما 300 بیمار دریافت کننده IVIG بستری در بخش‌های مختلف بیمارستان رازی و ولیعصر (عج) بیرجند طی سال 98-1395 بود. اطلاعات دموگرافیک، دوز، مدت زمان تجویز، طول مدت بستری، عوارض جانبی و سایر اطلاعات لازم از اطلاعات بیماران استخراج شد. دادههای جمع آوری شده با اندیکاسیون و دوز مورد تأیید FDA مقایسه و میزان تجویز منطقی دارو گزارش شد. داده‌ها در نرم‌افزار SPSS نسخه 19 وارد شد. برای مقایسه متغیرهای کمّی و کیفی در گروهها از آزمون T مستقل (یامن ویتنی) و کای دو استفاده شد. یافته‌ها: نتایج نشان داد که در این مطالعه 150 نفر (50 %) مرد بودند. میانگین سن بیماران دریافت کننده IVIG 01/27±17/28 سال بود. اغلب بیماران دریافت کننده IVIG در بخش نورولوژی (3/30%) بستری بودند. بیماری گیلن باره و پورپورای ترومبوسیتوپنیک ایمنی بیشترین موارد تجویز IVIG بودند. در این مطالعه 3/44 درصد (1/50- 6/3895%CI:) (133 بیمار) مطابق اندیکاسیون پذیرفته شده (FDA on-labeled) دارو دریافت کردند. دوز دریافتی IVIG براساس FDA در 33 درصد (6/38- 7/2795%CI:) موارد منطقی بود. نتیجه‌گیری: با توجه به دسترسی محدود و قیمت بالای این فرآورده، تدوین و نظارت دقیق بر اجرای مصرف منطقی IVIG در بیمارستان بر اساس پروتکل استاندارد منطقه‌ای به‌ویژه با حضور داروسازان بالینی پیشنهاد می‌گردد.</div> <div style="text-align: justify;">Background and Aims: Intravenous immunoglobulin (IVIG) is one of the safest biological products, which is effective in the treatment of many diseases. This study aimed to evaluate the rational prescription pattern of IVIG based on the approved indications of the Food and Drug Administration (FDA) among teaching hospitals in Birjand, Iran. Materials and Methods: This descriptive-analytical study included 300 patients receiving IVIG who were hospitalized in different wards of Razi and Vali-e-Asr hospitals in Birjand during 2016-19. Demographic characteristics, dose, duration of administration, duration of hospitalization, side effects, and other necessary information were extracted from patients' medical records. The collected data were compared with FDA-approved indications and dosages, followed by identifying the rate of rational prescription of the drug. The data were analyzed in SPSS software (version 19) through independent t-test (or Mann-Whitney) and Chi-square test to compare quantitative and qualitative variables in the groups, respectively. Results: According to the results, half of the patients were male (n=150; 150%). The mean age of the patients receiving IVIG was 28.17±27.01 years. Most of the patients receiving IVIG (30.3%) were hospitalized in the neurology ward. Guillain-Barre syndrome and immune thrombocytopenic purpura accounted for the most prescription of IVIG. In total, 133 patients (44.3%) (95%CI: 38.6-50.1) received the drug according to the FDA on-labeled indication. The dose of IVIG was rationally based on FDA in 33% of the cases (95%CI: 27.7-38.6).
