en تحلیل مفهوم پریشانی اخلاقی در تکنسین‌های اورژانس پزشکی: یک مطالعه کیفی Analyzing the concept of moral distress in emergency medical technicians: A qualitative study پرستاری- مطالعات کیفی Nursing- Qualitative Study مقاله اصیل پژوهشی Original Article <div style="text-align: justify;">زمینه و هدف: پریشانی اخلاقی مفهومی پیچیده است که در حیطهء خدمات پزشکی اورژانس، نیازمند شفاف‌سازی بیشتری می‌باشد. هدف مطالعه حاضر، واکاوی ابعاد گوناگون مفهوم پریشانی اخلاقی بود. روش تحقیق: مطالعهء حاضر کیفی و با رویکرد تحلیل مفهومی واکر و آوانت انجام شد. بازهء زمانی اجرای پژوهش دی‌ماه ۱۴۰۳ تا خرداد‌ماه ۱۴۰۴ بود. پایگاه‌های اطلاعاتی Web of Sciences، PubMed، Embase، Scopus و Google Scholar مورد جستجو قرار گرفتند. جستجو با کلیدواژه‌های «پریشانی اخلاقی»، «تردیدهای اخلاقی»، «نگرانی‌های اخلاقی»، «خدمات پزشکی اورژانس» و «اورژانس پیش‌بیمارستانی» انجام شد. پس از تحلیل داده‌ها، ویژگی‌های تعریف‌کننده، پیش‌نیازها و پیامدهای مفهوم استخراج شد. یافته‌ها: در فرایند جستجو ۱۵۲۱ مقاله شناسایی شد. پس از حذف موارد تکراری و مطالعات نامرتبط و نیز غربالگری متن کامل، نهایتاً ۲۵ مقاله در تحلیل نهایی مفهوم گنجانده شد. یافته‌ها نشان داد ویژگی‌های تعریف‌کننده پریشانی اخلاقی شامل «پدیدار شدن در محیط‌های بالینی پیچیده»، «مواجهه با معضل اخلاقی در رضایت آگاهانه»، «ناراحتی ناشی از تضعیف شأن حرفه‌ای» و «آگاهی از ضعف مبانی اخلاقی» بودند. پیامدهای آن نیز شامل «نارضایتی شغلی»، «انزوای عاطفی از بیماران»، «فرسودگی شغلی»، «کاهش کیفیت مراقبت» و «ترک شغل» بودند. نتیجه‌گیری: در این مطالعه، پریشانی اخلاقی «یک حالت ذهنی ناراحت‌کننده‌ای تعریف شده است که در موقعیت‌های بالینی پیچیده و غیرقابل‌پیش‌بینی، فرایند اخذ رضایت آگاهانه، درکِ شأن حرفه‌ای و مبانی اخلاقی تضعیف‌شده در تکنیسین‌های اورژانس پزشکی تبلور می‌یابد». این ویژگی‌ها می‌توانند در مطالعات کیفی و کمّی بعدی مورد توسعه و آزمون قرار گیرند تا به تکمیل و درک عمیقتر این مفهوم کمک کنند.</div> <div> <table align="left" hspace="0" vspace="0"> <tbody> <tr> <td style="padding: 0in 12px; text-align: justify;" valign="top">Background and Objective: Moral distress is a complex concept that requires further clarification in emergency medical services. The present study aimed to explore the different aspects of moral distress. Materials and Methods: The present study was qualitative and employed the Walker and Avant conceptual analysis approach. The research period was from January 2004 to June 2005. The databases of Web of Science, PubMed, Embase, Scopus, and Google Scholar were searched. The search was conducted using keywords "moral distress," "ethical doubts," "ethical concerns," "emergency medical services," and "prehospital emergency." After analyzing the data, the defining characteristics, prerequisites, and consequences of the concept were identified. Results: The search process identified a total of 1,521 articles. After removing duplicates, unrelated studies, and screening the full texts, 25 articles were ultimately included in the final analysis. The findings showed that the defining characteristics of moral distress included “occurrence in complex clinical settings,” “encountering an ethical dilemma in informed consent,” “discomfort due to weakening professional dignity,” and “awareness of weak ethical foundations.” Its consequences included “job dissatisfaction,” “emotional isolation from patients,” “burnout,” “decreased quality of care,” and “job abandonment”.
