fa شیوع هیپراستوز اسکلتی ایدیوپاتیک منتشر بر اساس سی‌تی‌اسکن قفسه سینه در بیماران مراجعه کننده به بیمارستان ولی‌عصر بیرجند، 1398-1397 Prevalence of diffuse idiopathic skeletal hyperostosis based on chest computed tomography scans, 2019-2020 راديولوژي Radiology مقاله اصیل پژوهشی Original Article <div style="text-align: justify;">زمینه و هدف: هیپراستوز اسکلتی ایدیوپاتیک منتشر (DISH) اختلالی متابولیک و غیرالتهابیِ ستون فقرات است که با استخوانی‌شدن پیشرونده رباط‌ها مشخص می‌شود و با افزایش سن شیوع بیشتری می‌یابد. از آنجا که سی‌تی‌اسکن قفسه سینه در بسیاری از بیماران به دلایل غیرارتوپدی انجام می‌شود، استفاده فرصت‌جویانه از این تصاویر می‌تواند برآورد دقیق‌تری از شیوع ارائه کند. هدف این مطالعه، تعیین شیوع و الگوی درگیری توراسیک DISH بر پایه سی‌تی قفسه سینه در یک مرکز ارجاعی بود. روش تحقیق: در مطالعه مقطعیِ گذشته‌نگر حاضر، تصاویر سی‌تی قفسه سینه ۱۰۰۰ بیمار ≥۱۶ سال که از مارس ۲۰۱۹ تا فوریه ۲۰۲۰ در بیمارستان ولی‌عصر بیرجند تحت سی‌تی‌اسکن قرار گرفته بودند، به‌صورت گذشته‌نگر بازبینی شد. پس از اعمال معیارهای خروج، ۹۴۹ پرونده تحلیل شد. تشخیص بر اساس معیارهای رزنیک–نیوایاما و با بازبینی مستقل دو رادیولوژیست انجام گرفت. یافته‌ها: از ۹۴۹ بیمار، ۱۲۵ نفر مبتلا به DISH بودند (۱۳٫۱۸٪؛ ۹۵٪CI: ۱۱٫۱–۱۵٫۴). میانگین سن مبتلایان ۷۰٫۸±۱۳٫۸ سال و بالاتر از افراد فاقد بیماری بود (۶۶٫۲±۱۰٫۵؛ 001/0>P). شیوع در مردان ۱۶٫۴٪ و در زنان ۱۰٫۲٪ بود. اضافه‌وزن (9/29–25: BMI) در ۶۴٪ موارد دیده شد. شایع‌ترین بیماری‌های همزمان، پرفشاری خون (۳۴٫۴٪)، دیس‌لیپیدمی (۲۴٫۰٪) و دیابت (۲۰٫۰٪) بودند. کمردرد (۵۸٫۴٪) و علائم سازگار با تنگی کانال نخاعی (۵۷٫۶٪) غالب بودند. درگیری T5–T8 در ۱۰۰٪، T9–T12 در ۹۵٫۲٪ و T1–T4 در ۴۷٫۲٪ مشاهده شد؛ شکستگی مهره‌ای ۱۵٫۲٪ و کلسیفیکاسیون لیگامان طولی خلفی ۴٫۰٪ بود. نتیجه‌گیری: شیوع DISH توراسیک بر پایه سی‌تی قفسه سینه 18/13 % بود و با سن بالاتر و جنس مذکر ارتباط داشت. گزارش فرصت‌جویانه DISH در قرائت سی‌تی قفسه سینه و ارزیابی عوامل خطر کاردیومتابولیک، همراه با ارجاع در صورت وجود علائم فشاری توصیه می‌شود.</div> <div> <div> <table align="left" hspace="0" vspace="0"> <tbody> <tr> <td style="padding: 0in 12px; text-align: justify;" valign="top"><a name="_Hlk200355634">Background and Aims:</a> Diffuse idiopathic skeletal hyperostosis (DISH) is a non-inflammatory metabolic disorder of the spine characterized by progressive ligament ossification and increasing prevalence with age. Since chest CT is commonly performed for non-orthopedic indications, opportunistic assessment may provide a more accurate prevalence estimate. This study aimed to determine the prevalence and thoracic involvement pattern of DISH on chest CT at a referral hospital in Birjand, Iran. Materials and Methods: In this retrospective cross-sectional study, chest CT scans of 1,000 patients aged ≥16 years who had undergone imaging at Vali-Asr Hospital, Birjand, South Khorasan, Iran, between March 2019 and February 2020 were retrospectively reviewed. After exclusions, 949 records were analyzed. DISH was diagnosed using the Resnick–Niwayama criteria with independent review by two radiologists. Results: Among 949 patients, 125 cases had DISH (13.18%; 95% CI: 11.1–15.4). Mean age was higher in affected patients (70.8±13.8 years) than in those without DISH (66.2±10.5; P<0.001). Prevalence rates were 16.4% and 10.2% in males and females, respectively. Overweight (BMI 25–29.9) was present in 64% of cases. The most common comorbidities were hypertension (34.4%), dyslipidemia (24.0%), and diabetes (20.0%). Back pain (58.4%) and symptoms suggestive of spinal canal stenosis (57.6%) predominated. Thoracic involvement occurred at T5–T8 in 100%, T9–T12 in 95.2%, and T1–T4 in 47.2%. Vertebral fracture was observed in 15.2%, and posterior longitudinal ligament calcification in 4.0%. Conclusion: Thoracic DISH prevalence on chest CT was 13.18% and was associated with older age and male gender.
