<p align="right"> آلرژی غذایی به واکنشهای غیرطبیعی بدن اطلاق میشود که به واسطه پاسخ سیستم ایمنی به اجزای غذایی ایجاد میگردند. در دهههای اخیر آلرژی به عنوان یک مشکل تهدیدکننده سلامت عمومی مطرح گردیده است. شیوع آلرژی غذایی در کودکان تا 10% تخمین زده شده است؛ این میزان در افراد بالغ کمتر از 2% میباشد. این شرایط در کشورهای مختلف دنیا متفاوت بوده و تا حدود زیادی از فرهنگ غذایی مردم تأثیر میپذیرد. آلرژیزاهای غذایی، پروتئینها و یا گلیکوپروتئینهایی با وزن مولکولی بین 10 تا 70 کیلودالتون میباشند. آلرژیزاها بر اساس نحوه تحریک سیستم ایمنی به دو گروه تقسیم میشوند: نوع اول به دلیل تحریک سیستم ایمنی (پس از صرف مواد غذایی) باعث ایجاد پاسخ علیه مواد غذایی در روده میشوند و نوع دوم دارای واکنش متقاطع (تشابه ساختمانی) با آلرژیزاهای موجود در هوا (اغلب گرده گیاهان) میباشند؛ بنابراین در نوع دوم، مرحله اول حساسسازی افراد، برخورد با آلرژیزای استنشاقی میباشد. در مرحله بعد چنانچه فرد از ماده خوراکی که تشابه آنتیژنی با آلرژیزاهای استنشاقی دارد، استفاده نماید، این ماده خوراکی باعث ایجاد آلرژی میشود. شکلگیری یک پاسخ آلرژیک نیز، اغلب، با تولید IgE در بدن در مواجهه با آلرژیزای غذایی آغاز میگردد. IgE اختصاصی به سلولهای ماستوسیت و یا بازوفیل متصل و باعث دگرانولاسیون آنها در تماسهای بعدی با ماده آلرژیزا میشود. پیامد این فرایند، رها شدن تعداد فراوانی از واسطهها از جمله هیستامین میباشد. این واسطهها میتوانند باعث ایجاد علائم بالینی آلرژی مانند کهیر، خارش و شوک آنافیلاکسی شوند. تشخیص آلرژیهای غذایی با روشهای مختلف درونتنی و برونتنی صورت میگیرد که میتوان به گرفتن شرح حال از بیمار، معاینه فیزیکی، تست پریک، حذف غذاهای مشکوک از رژیم بیمار و انواع تستهای چالش اشاره نمود. از جمله متداولترین درمانها که برای آلرژیهای غذایی به کار گرفته میشوند، حذف ماده غذایی آلرژیزا از رژیم غدایی بیمار، درمانهای دارویی، ایمونوتراپی و درمان با مکملهای غذایی میباشد. در سالهای اخیر نیز استفاده از آلرژیزاهای غذایی نوترکیب با کمک مهندسی ژنتیک امکان تشخیص انواع آلرژیها را با حساسیت بالایی فراهم ساخته است.</p>
Food allergy can be defined as adverse, immunomediated reactions to foods that occur in certain individuals. Food allergy is being increasingly recognized as a public health problem over the last few decades. It has been estimated that up to 10% of children and less than 2% of adults suffer from some type of food allergy. The situation varies in different countries depending on dietary eating pattern. Usually food allergens are proteins or glycoproteins with a molecular weight ranging from 10-70 kDa. Depending on the route of sensitization, food allergy is the result of either genuine reactivity to comestibles through the gastrointestinal tract (class I food allergens) or secondary sensitization to cross-reactive food allergens as a consequence of the initial reactivity to homologous pollen-related allergens (class II food allergens). An allergenic reaction occurs after an initial sensitization to a food allergen where IgE antibodies are produced. Subsequent exposures to the allergen may result in activation/degranulation of mast cells and basophiles (due to cross linking of IgE antibodies by allergens) and release biological mediators (such as histamine) that manifest in observable reaction such as hives, rashes, and anaphylactic shock. Diagnosis of food allergies could be based on in vitro or/and in vivo tests such as clinical history, physical examination, Prick test, elimination of suspected foods from the patient’s diet and different types of food challenge tests. The elimination of food allergens from the diet, pharmacological treatment, immunothrapy and supplementary treatments are the most common treatments. In recent years, the use of recombinant food allergens recruiting genetic engineering, allows highly sensitive detection of sensitization profiles.
