بررسی کارایی و تکرار پذیری سیستم سیدنی در ارزیابی و درجهبندی گاستریت مزمن

<p style="text-align: justify;"> <strong> زمینه و هدف </strong>: گاستریت مزمن یکی از شایعترین اختلالات گوارشی است و یافتن علت آن بخصوص هلیکوباکترپیلوری و عوامل مستعدکننده بدخیمی (آتروفی، متاپلازی رودهای) تا حدّ زیادی بر پایه بافتشناسی است؛ بنابراین سیستمی که بتواند از دقت و تکرارپذیری مناسبی برای ارائه مشاهدات برخوردار باشد، بسیار مهم است. سیستمی که امروزه به طور رایج برای ارزیابی و درجهبندی گاستریت مزمن به کار میرود، Updated Sydney میباشد. مطالعه حاضر با هدف بررسی میزان توافق بین مشاهدهکنندگان در مورد هر کدام از معیارهای این سیستم و به عبارتی بررسی میزان تکرارپذیری مشاهدات انجام شد.</p>
<p style="text-align: justify;"> <strong> روش تحقیق: </strong>در این مطالعه توصیفی- تحلیلی، 44 نمونه بیوپسی آندوسکوپی معده با رنگآمیزی هماتوکسیلین و ائوزین (H&E) و گیمسا توسط دو نفر متخصص آسیبشناسی به طور مجزا مورد بازبینی قرار گرفت. هر متخصص آسیبشناسی برای معیارهای سیستم سیدنی یعنی شدت التهاب (آماس) مزمن و تراکم (دانسیته) هلیکوباکترپیلوری (H.P) به صورت 0-3 (عدم، خفیف، متوسط و شدید) و آتروفی غددی و متاپلازی رودهای به صورت 10 (وجود، فقدان) نمرهای تعیین نمود. میزان ارتشاح نوتروفیلی (دالّ بر فعالیت بیماری) نیز به صورت 0-3 بررسی شد. نتایج حاصل از مشاهدات با استفاده از محاسبه ضریب K و آزمون آماری McNemar مورد تجزیه و تحلیل قرار گرفتند.</p>
<p style="text-align: justify;"> <strong> یافتهها </strong>: بیشترین میزان توافق در مورد متاپلازی رودهای (81/0= K ) مبنی بر توافق عالی بود. در مورد شدّت التهاب مزمن (67/0= K ) ، تراکم H.P (62/0= K ) و ارتشاح نوتروفیلی (68/0= K ) توافق خوبی وجود داشت. کمترین میزان توافق در مورد آتروفی غددی (32/0= K ) بود.</p>
<p style="text-align: justify;"> <strong> نتیجهگیری </strong>: سیستم سیدنی، سیستم مفیدی برای ارزیابی و درجهبندی گاستریت مزمن است و در بیشتر موارد توافق خوبی بین مشاهدهکنندگان وجود دارد؛ این سیستم از تکرار پذیری مناسبی برخوردار است. به نظر میرسد عدم توافق مناسب در مورد آتروفی ناشی از تفاوتهای فردی در ارزیابی و فقدان معیارهای تشخیصی استاندارد برای وجود و شدّت آن باشد.</p>
<p style="text-align: justify;"> <strong> Background and Aim: </strong>Chronic gastritis is one of the most common gastrointestinal disorders and detection of its etiology especially Helicobacter pylori and risk factors of malignancy (atrophy and intestinal metaplasia) is, to a great extent, based on histological assessment. Therefore, a system which could sustain good reproducibility in reporting observations is very important. Today, the most frequently used system for assessment and grading of chronic gastritis is the Updated Sydney System. The aim of this study was to find out inter-observer agreement on each criterion of this system (i.e reproducibility rate ).</p>
<p style="text-align: justify;"> <strong> Materials and Methods: </strong>In this descriptive analytical study, 44 endoscopic gastric biopsy samples (with hematoxylin, eosin, and geimsa stains) were reviewed by two pathologists in turn. Each pathologist graded any of Sydney System criteria including chronic inflammation and H.P density level from 0-3 (absent, mild, moderate and severe) glandular atrophy and intestinal metaplasia from 0-1 (absence and presence). In addition, neutrophilic infiltration (activity of disease) was also graded from 0-3. Finally results of the observation were analaysed by means of McNemar statistical test and K coefficient</p>
<p style="text-align: justify;"> <strong> Results: </strong>The most/best inter-observer agreement was found on intestinal metaplasia ( K =0.84) representing. Good interobserver agreement was observed on chronic inflammation ( K =0.67), H.P density (K=0.62) and neutrophilic infiltration (K=0. 68). For glandular atrophy, agreement was the least (K=0.32)</p>
<p style="text-align: justify;"> <strong> Conclusion: </strong>Updated Sydney system is a useful system for the assessment and grading of chronic gastritis. In most cases there was good agreement between the observers. Reproducibility is evaluative differences in grading a specific feature and lack of standardized diagnostic criteria to charactrize its presence and degree.</p>
