فراوانی علل تشنج در کودکان بستری بیمارستان تخصصی اطفال زاهدان

زمینه و هدف: تشنج در کودکان با شیوع 4-6 مورد در 1000 کودک، شایعترین اختلال نورولوژیک طب کودکان است. با توجه به شیوع نسبتا بالای تشنج و اهمیت اثرات روانی - اجتماعی و شناختی آن، مطالعه حاضر با هدف تعیین علل و عوامل زمینه ای تشنج در کودکان انجام شد.روش تحقیق: در این مطالعه توصیفی - تحلیلی، 178 کودک بستری مبتلا به تشنج با روش سرشماری انتخاب شدند. داده ها با استفاده از پرسشنامه سه قسمتی شامل مشخصات فردی، حمله تشنجی و رویه های تشخیصی انجام شده، جمع آوری گردید؛ سپس با استفاده از نرم افزار SPSS و آزمون آماری Chi-Square و ضریب همبستگی در سطح معنی داری P£0.05 مورد تجزیه و تحلیل قرار گرفتند.یافته ها: از 178 کودک مورد بررسی، %42.8 مونث و %57.2 مذکر بودند. %86.7 با زایمان طبیعی و %13.3 با سزارین متولد شده بودند. میانگین طول بستری %5.63 روز، سن کودک 3.05 سال، وزن هنگام تولد 2838.92 گرم و میانگین طول مدت تشنج 7.85 دقیقه بود. دفعات تشنج در %44.4 کودکان، سه بار در روز و در %20.2 آنان سابقه خانوادگی، مثبت بود. %48.6 بیماران تشنج فیبریل، %28.1 صرع، %5.6 عفونت، %9.6 انسفالوپاتی استاتیک و %7.9 سایر علل تشنج را داشتند. بین جنس کودک و نوع زایمان و وزن تولد با علت تشنج ارتباط معنی داری وجود نداشت ولی با سن بروز اولین حمله تشنجی و طول مدت تشنج و دفعات تشنج ارتباط معنی داری حاصل گردید (P=0.00). عوامل زمینه ای تشنج ناشی از تب، عفونت تنفسی فوقانی (%40)، گاستروآنتریت (%3.4)، عفونت ادراری (%4)، پنومونی (%12)، اوتیت (%5)، سپتی سمی (%3.6) و تب با منشا ناشناخته (%5) بودند.نتیجه گیری: گرچه تشنج در بیشتر موارد اختلال خوش خیمی محسوب می شود، فراوانی آن دسته از تشنج ها که نیازمند بررسی، تشخیص و برنامه ریزی درمانی ویژه هستند، آن قدر هست که در مورد هر کودک شرح حال کامل، معاینه دقیق و ارزیابی پاراکلینیک انجام شود؛ زیرا ارزیابی تشخیصی روی تصمیمات درمانی، مشاوره خانوادگی، نیاز به بستری شدن و پیگیری خاص این بیماران تاثیر می گذارد. ارایه شواهد دقیق از حملات در طول زمان با شناسایی عوامل ایجاد کننده و تشدید کننده حملات کودک می کند و پرستار با کمک آن می تواند دفعات حمله را کاهش دهد یا از آن جلوگیری کند و از بروز عوامل آن بکاهد.
<p> <strong> Background and Aim: </strong>Convulsion in infants with the prevalence of 4 to 6 cases in 1000 is the most common neurological disorder in pediatrics. The reason for this study was the rather high prevalence of convulsion, its psychosocial and cognitive importance, and determining the underlying factors of the disorder in infants.</p>
<p> <strong> Materials and Methods: </strong>In this descriptive- analytical study, 178 hospitalized infants suffering from convulsion were selected through purpose- based sampling. Data gathering was done by means of a three-section questionnaire which included personal characteristics, convulsion seizure, and diagnostic procedures performed .Then, the obtained data was analysed employing SPSS software, using chi-square, and correlation coefficient statistical tests at the significant level P≤0.05.</p>
<p> <strong> Results: </strong>Out<strong> </strong>of 178 infants studied, 57.2% were males and 42.8% females. 86.7% had been born naturally and 13.3% through cesarean section. Mean hospitalized time was 5.63 days, €mean child age was 3.05 years, birth weight was 2838.92gr, and mean convulsion time was 7.85 minutes. Frequency of convulsion in 44.4% of the children was 3 times a day and 20.2% of them had a positive familial history 48.6% of the patients had febrile convulsion, 28.1% epilepsy, 5.6% infection, 9.6% static encephalopathy , and 7.9% revealed other factors of convulsion. There was not a significant relationship between either sex , delivery type, or birth weight to convulsion but the relationship to age at the onset of convulsion, its period and frequency was significant (P=0.00). Underlying factors of convulsion were fever, upper respiratory infection (40%), gastroenteritis (3.4%), urinary infection (4%), pneumonia (12%), otitis (5%), septicemia (3.6%), and unidentified fever (5%).</p>
<p> <strong> Conclusion: </strong>Although convulsion is a benign disorder in most cases, the frequency of those convulsions which require investigation and special treatment planning is of such level that complete description, careful examining and paraclinical assessment planning should be performed for each infant because diagnostic assessment influences treatment, family counseling, necessity of hospitalization, and particular follow-up of these patients. Presenting accurate reports of convulsion attacks will help the nurse to identify underlying and intensifying factors, reduce or prevent their frequency and decrease potential complications.</p>
