تاثیر درمان با لووتیروکسین بر سطح آنتی بادی تیروئید پراکسیداز در بیماران زن 20-50 ساله مبتلا به هاشیموتو

زمینه و هدف: تیروئیدیت هاشیموتو، شایع ترین علت هیپوتیروئیدیسم در کودکان و بزرگسالان است. تظاهر کلینیکی بیماری به سه شکل هیپوتیروئید، یوتیروئید و به طور نادر هیپرتیروئید می باشد. مطالعه حاضر با هدف تعیین تغییرات سطح آنتی بادی ضد تیروئید پروکسیداز (anti-thyroid peroxidase: anti-TPO) قبل و بعد از درمان با لووتیروکسین در بیماران تیروئیدیت هاشیموتو انجام شد.روش تحقیق: در این مطالعه نیمه تجربی، ابتدا بیماران زن مراجعه کننده به درمانگاه غدد کسری بیرجند در سال 85-1384 که گواتر با قوام (Firm) داشتند و سن آنان بین 20-50 سال بود، انتخاب شدند و در صورت مثبت بودن آنتی بادی anti-TPO تشخیص هاشیموتو برای آنها در نظر گرفته شد؛ سپس این افراد تحت دوره درمان سه ماهه با قرص لووتیروکسین به میزان 100 میکروگرم روزانه قرار گرفتند؛ در پایان سه ماه، سطح آنتی بادی anti-TPO آنها به همراه وضعیت بالینی، ارزیابی گردید. داده ها با استفاده از نرم افزار آماری SPSS و آزمون های آماری t مستقل، t زوج و Chi-Square مورد تجزیه و تحلیل قرار گرفتند.یافته ها: 67.1% از بیماران، هیپوتیروئید و 32.9% یوتیروئید بودند؛ سطح آنتی بادی anti-TPO در 20% از بیماران، پس از درمان با دوره سه ماهه لووتیروکسین، به حد طبیعی رسید. میانگین سطح آنتی بادی anti-TPO قبل از درمان با لووتیروکسین 450.74±122.05 IU/mL بود که پس از درمان به 236.2±153.96 IU/mL کاهش یافت؛ این کاهش از نظر آماری معنی دار بود (P<0.001). میانگین سطح آنتی بادی anti-TPO قبل از درمان در هیپوتیروئیدها نسبت به افراد یوتیروئید بالاتر بود (P<0.00). بعد از درمان با لووتیروکسین، سطح آنتی بادی در افراد هیپوتیروئید بالاتر از افراد یوتیروئید باقی ماند (P<0.001).نتیجه گیری: سطح آنتی بادی anti-TPO در بیماران هاشیموتو که هیپوتیروئید هستند، بالاتر از بیماران یوتیروئید است. بعد از درمان، سطح آنتی بادی anti-TPO در هر دو گروه کاهش می یابد ولی این کاهش در افرادی که یوتیروئید می شوند، بیشتر از افرادی است که هیپوتیروئید باقی می مانند؛ با توجه به این مطالب می توان اذعان داشت که اندازه گیری آنتی بادی anti-TPO به عنوان روشی مهم برای تشخیص و پیگیری درمان و ارزیابی پیش آگهی بیماری هاشیموتو، قابل استفاده خواهد بود.
Background and Aim: Hashimoto's thyroiditis is the most prevalent cause of hypothyroidism in children and adults. The disease clinically manifests itself in three forms namely hypothyroidism, euthyroidism, and hyperthyroidism. The present study was carried out aiming at determining anti-thyroid peroxidase antibody variation before and after levothyroxine treatment in hashimoto's thyroiditis patients. Materials and Methods: In this quasi-experimental study, at first patients with firm goiter referring to Birjand glands deficiency clinic between 2005 and 2006 were examined, and those with positive anti- TPO antibody were accounted as hashimoto cases. Then, these cases were prescribed a three- month treatment with 100 microgram daily of levothyroxine. At the end of the period, their level of anti- TPO antibody was measured and their clinical conditions were assessed. The obtained data was then analysed by means of SPSS software statistical tests including independent t, paired t test and chi-square test. Results: Majority (67.1%) of the patients were diagnosed as hypothyroid, and 32.9% were euthyroid cases. The level of anti-TPO antibody in 20% of the patients reached normal level after a period of three month treatment with levothyroxine. Mean anti-TPO antibody level before levothyroxine treatment was 450.74±122.05 IU/mL, which reduced to 236.2±153.96 IU/mL after treatment (P<0.001). Mean anti-TPO antibody level before treatment was higher (P<0.001) in hypothyroid patients compared with euthyroid ones. After treatment with levothyroxine, antibody level in hypothyroid cases also remained higher than that of euthyroid ones (P<0.001). Conclusion: Anti-TPO antibody level in Hashimoto patients with hypothyroidism is higher than euthyroid ones. After treatment, anti-TPO antibody level decreases in both groups but the decrease is more prominent in euthyroid cases than patients who remain hypothyroid. Regarding these facts, it could be confirmed that anti-TPO antibody measurement (as an important diagnostic and follow-up procedure and for prognosis in Hashimoto) will be useful.
