سطوح شواهد بالینی در مقالات منتشر شده در مجلات علمی- پژوهشی علوم پزشکی ایران (1387-1373)

<p> <strong> زمینه و هدف: </strong>پزشکی مبتنی بر شواهد بر استفاده صحیح از نتایج تحقیقات علمی بر بالین بیمار تأکید دارد. در این مطالعه به ارزیابی سطوح شواهد بالینی مقالات منتشرشده در مجلات علمی- پژوهشی علوم پزشکی ایران در یک دوره پانزده ساله (1387-1373) پرداخته شده است.</p>
<p> <strong> روش تحقیق: </strong>در این مطالعه توصیفی، با روش نمونهگیری تصادفی منظم، سال‏های 1376، 1381 و 1386 به عنوان نمونه انتخاب و مقالات بالینی منتشرشده در شمارههای فرد مجلات علمی- پژوهشی علوم پزشکی کشور در آن سال‏ها استخراج شدند. این مقالات بر اساس جدول سطوح شواهد بالینی آکسفورد، از سطح یک (بالاترین) تا سطح چهار (پایینترین)، رتبهبندی شدند. برای تحلیل و مقایسه روند انتشار مقالات، از نرمافزار SPSS (ویرایش 5/11) و آزمون مجذور کای در سطح معنیداری 05/0>P استفاده شد.</p>
<p> <strong> یافتهها: </strong>از 4497 مقاله منتشرشده در سه سال منتخب، تعداد 2860 مقاله بالینی استخراج و سطوح شواهد آنها تعیین شد. مجموع مقالات سطوح یک تا سه از 9/19% در سال 1376 به 1/30% در سال 1386 افزایش داشته است که روند رو به رشد شواهد بالینی را در طیّ زمان نشان میدهد (001/0=P). میانه سطوح شواهد در دوره مطالعه از سطح سه به سطح دو تغییر یافته است. بیشتر مقالات بالینی (7/59%) در هیچ‏یک از چهار سطح درجهبندی شواهد طبقهبندی نشدند.</p>
<p> <strong> نتیجهگیری: </strong>سطح شواهد مقالات بالینی مجلات علمی- پژوهشی علوم پزشکی ایران در طیّ دوره مطالعه به طور معنیداری افزایش داشته است؛ بنابراین به نظر می‏رسد کیفیت تحقیقات بالینی در ایران بر اساس استاندارد پزشکی مبتنی بر شواهد، روند رو به رشدی را پیموده است.</p>
<p> <strong> Background and Aim: </strong>Evidence Based Medicine (EBM) emphasizes on proper usage of the results of scientific research in the bedside for patients. This study evaluated the levels of clinical evidence in published articles in Iranian scientific medical journals over a fifteen-year period (1994-2008).</p>
<p> <strong> Materials and Methods: </strong>In this descriptive study, the years 1997, 2002 and 2007 were selected using systematic random sampling method and all of the clinical articles published in odd numbers of Iranian medical journals in these 3-year were analyzed. These articles were categorized according to the Oxford Centre for EBM from level I (highest) to IV (the lowest). Data were analyzed by means of SPSS (version 11.5), using chi-square test at the significant level of P<0.05.</p>
<p> <strong> Results: </strong>Out of 4497 published articles in the selected 3-year, 2860 clinical articles were determined and evaluated. The total number of articles with levels I- III increased from 19.9% in year 1997 to 30.1% in year 2002 which indicate a significant growing of clinical evidence over the time (P=0.001). The median level of clinical evidence rating improved from 3 to 2 during the study period. The majority of clinical articles (59.7%) were classified as non-evidence.</p>
<p> <strong> Conclusion: </strong>The level of evidence of clinical articles in Iranian scientific medical journals has improved significantly during the studied period. Therefore, it seems the quality of clinical research in Iran has been improved according to standards of EBM.</p>
